Non-Public School Forms

As a service to all authorized school administrators the Office of Funded Programs (OFP) has made Federal, State and District forms accessible and available.

Please note the following:

• Mentor applications and mentor time sheets must be hand delivered (with original signatures) to the Office of Funded Programs.

• Order request forms must first be approved by the OFP and then used to generate a purchase order. Order request forms are never to be sent directly to any vendor.

• The appropriate school administrator must authorize all forms.

• Forms with missing information or duplicate forms will be returned to the school.

Important Dates and Deadlines for the 2006-07 School Year

There will be no exceptions or changes to these deadlines

·        Title I – May 15, 2007

·        Title IIA – May 15, 2007

·        Title IID – May 15, 2007

·        Title III – May 15, 2007

·        Title IV – May 15, 2007

·        Title V – May 15, 2007

·        Purchase Requests for Computer Software, Library Books and Text books for the 2007-2008 School Year must be submitted between July 1, 2007 and August 31, 2007

Mentor-Tutor Title I & III Staff

§        The last Board Meeting of the 2006-07 school year is June 20.  To ensure approval of summer services(2007), all changes of mentor status must be submitted by June 4th.  There is no guarantee that there will be a board meeting during July and August 2007.   Please consider this when planning your summer program.
