Office of Central Registration District-Wide Registration Fair On Tuesday, August 15th and Tuesday, August 22, East Ramapo will hold a registration fair for parents whose children will be attending East Ramapo schools during the 2006-2007 school year. Registration will be held in the Central Registration Office, at 105 S. Madison Ave, Spring Valley, from 10 am until 6 pm. by coming to this fair on either of these days, you can complete the registration process in one location and avoid the very long lines that occur in early September. Click here for driving directions In order to register a child for school you will need the documentation noted below: Please make sure to bring it with you. Proof of Birth (one of the following)
Original Birth Certificate with raised seal Copy of Birth Certificate with raised seal
Original B Passport Green Card
Proof of Immunizations
Statement signed by a physician or clinic listing the month, day and year of the following immunizations including:
2 doses MMR (measles/mumps/rubella) 3 doses DPT(diptheria) 3 doses IPV or OPV polio vaccination if born on or after January 1, 1993 - 3 does Hepatitis B vaccine if born on or after January 1, 1998 - 1 Dose Varicella (Chicken Pox)
Proof of Residency (Two of the following - MUST BE CURRENT)
Lease Mortgage papers United Water Bill Telephone (not wireless) Bill Orange & Rockland Bill Current Driver's License Notarized rent receipt with name and address of tenant and landlord
School Records
the name and address of the current or last school the child attended the child's last report card the child's IEP, if appropriate
NOTE: Only a parent or guardian can sign in a child.
